We are open Monday through Thursday 9:00AM to 5:00PM and closed most holidays.
From the North on I-15:
Take EXIT 307 toward 400 S. Keep LEFT at the fork to go on 400 S. Turn LEFT on 400 E.
From the South on I-15:
Take EXIT 306 (600 So.). Stay STRAIGHT to go onto UT-269 E/ 600 S. Continue to follow 600 S Turn LEFT onto S 400 E.

For those coming from out of town, our patient coordinator would be more than happy to give you a healthy recommendation to meet your dietary needs.
